Options Counseling

We can offer information and resources to help meet people’s long-term care needs. Options counselors also discuss factors to consider when making long-term care decisions. These might include but are not limited to housing, home care, home-delivered meals, personal response systems, and medication management.

ADRC is the place to go to learn about Long Term Care Programs, including eligibility requirements, and to start the application process. We perform the Wisconsin Adult Long-term Care Functional Screen, which is used to collect information about your ability to complete daily activities of living and how health issues affect your abilities. The functional screen is one determinate of eligibility and gets your application started.

What can an Options Counselor help with?

  • Resources to help
  • Medicaid applications, if planning to enroll in long term care.
  • Assessments for eligibility for long-term care programs.
  • Enrollment into long-term care program.
  • Meet with individuals and/or families to discuss options for people to remain in their homes or discuss alternative residential options.

Who can receive Options Counseling Services?

  • Chippewa County Residents
    • Frail Elders (65 and older)
    • Adults with physical and/or development disabilities

Is there a cost?

  • No, ADRC services are free.
