If you are a young adult transitioning from high school, or you need to transition from children’s programs to adult programs, we can help you make a smooth transition. The Options Counselors and the Disability Benefit Specialist at our office can provide information on:
- Places you can live
- Supports available to get a job and/or keep a job
- Social Security benefits such as SSI (Supplemental Security Income)
- Types of Medicaid and how to apply
- Adult Long-Term Care Medicaid programs such as Family Care and IRIS
- Ways to save money that won’t affect eligibility for Medicaid, SSI or other public benefits
- Surrogate decision-making options (guardianship, powers of attorney, supported decision making)
- Mental health services and mental health programs
When You Should Contact the ADRC
- You can contact us at any age to ask questions and get information on community resources and services
- At age 17.5 years, the ADRC can help determine financial and functional eligibility for adult Long-Term Care Medicaid programs called Family Care and IRIS
