STATEWIDE ISSUANCE

With the statewide issuance law now in effect, if the vital record you are requesting falls within the dates listed below you can obtain the copies from any Register of Deeds office in the State of Wisconsin.

Death
09/01/2013  Present
Birth, Marriage, and Death Records prior to the above dates contact the Register of Deeds in the County where the event occurred.
Divorce 01/01/2016  Present
Divorce Records prior to 01/01/2016 contact the Clerk of Courts in the County where the divorce occurred.


 

IN PERSON REQUESTS:
  • Processed while you wait
  • Bring your valid Photo ID
  • If possible complete the PDF application below, print it out and bring it in
  • To speed up the process, call ahead to get your record updated
  • As of January 3, 2023, any vital record request received after 4:15 pm that needs to be updated by the State Vital Records Office may not be completed until 8:15 am the following business day.

MAIL IN REQUESTS:
  • You may receive the record in one to five days from the date we receive your mailed in request
  • Send the completed form to Chippewa County Register of Deeds, 711 N Bridge St, Chippewa Falls, WI 54729, along with the appropriate payment
  • Be sure to include a copy of your valid Photo ID
